St. Martin's may use my child's photograph or artwork in church publications, bulletin boards, and/or website. (The child's name will not be used on the website.)
Medical Emergencies
In the event my child needs medical treatment and I cannot be reached or emergency medical treatment is needed, I give permission to St. Martin's Lutheran Church staff to secure medical treatment for my child.
Transportation*
My child has permission to travel with approved drivers (per SMLC CYF Policies - this policy is available from the office) to and from a specific event.

My child has a SERIOUS/LIFE THREATENING ALLERGY. If yes, please complete the "Allergy Action Plan" form and return it to the church office.
